The biomechanical advantage of rigid cross-arch fixation by dental implants coupled with the functional significance of treating edentulous ‘oral invalids’ often meant a favorable risk-to-benefit ratio and sound prospects of success for full-arch restorations. But the trend has been to use less and less implants in total rehabilitation, and immediately load them. In doing so, are we pushing the limits, or simply providing the product of evolution in this field?
We have long passed the development phase of implant therapy where restoring function and ensuring implant ‘survival’ were central. In today’s society, these criteria alone are no longer acceptable. Function, aesthetics and long-term oral health parameters must all go hand in hand as implant ‘Survival’ is not necessarily synonymous with Implant ‘Success’, just as a cosmetic procedure is not necessarily an aesthetic one. We know the implants will survive; we now need to make them successful.
Whilst dexterous accuracy and following surgical and prosthetic protocols are paramount in implant therapy, these will always be secondary to sound treatment planning. As clinicians in the era of ‘aesthetics and high expectations’, we must think beyond the ‘teeth’.
The modern challenge of total rehabilitation lies in our ability to improve function and aesthetics while at the same time facilitating long-term oral health and improving the longevity of our superstructures.
Biography
Dr Alex Fibishenko heads the implant clinic at the Centre for Aesthetic & Implant Dentistry and Australian All-On-4 plus network in Melbourne. Having graduated from the University of Melbourne in 1995, he pursued his interest in oral implantology and grafting and received training from some of the pioneers and leaders in the field in North America, Europe, Asia and Australia. Fellow of International Congress of Implantologists (ICOI) and Chairman of Im- plant Team Academy ITA, Dr Fibishenko’s practice is exclusive to implant surgery and plastic reconstructive oral surgery. He is credited with numerous advanced surgical techniques used today to improve the outcome of oral rehabilitation with dental implants. He works closely with selected dentists and specialists in Victoria and NSW, and is considered by many of his peers as one of Australia’s most progressive and leading experts in this area.
